Pay: $55.00 - $65.00 per hour

Location: Ramona, CA - Hybrid. This is a field-based home health position serving patients throughout Ramona, Poway, Rancho Bernardo, Rancho Peñasquitos, and surrounding communities.
Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.

Job Description
* Evaluate patients in their homes and assess mobility, strength, balance, coordination, pain, and functional limitations.
* Develop individualized physical therapy treatment plans based on each patient's needs and goals.
* Provide therapeutic exercises and techniques designed to improve mobility, strength, function, and independence.
* Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
* Monitor patient progress and adjust treatment plans as appropriate.
* Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team.
* Complete accurate and timely clinical documentation.
* Travel throughout the assigned North County San Diego territory to provide high-quality in-home care.
Qualifications
* Active California Physical Therapist (PT) license required.
* Home health Physical Therapist experience preferred.
* 1+ year of Physical Therapist experience preferred.
* Master's or Doctorate degree in Physical Therapy preferred.
* Current BLS or hands-on CPR certification preferred.
* Valid driver's license and reliable insured vehicle preferred.
* HCHB/Homecare Homebase experience preferred.
* OASIS experience preferred.
