Home Infusion RN - Patient Care & Coordination

Naven Health is seeking an Infusion Nurse II to coordinate and deliver intermediate care to patients in their homes or infusion suites, under a physician's plan of care. You will collaborate with a multidisciplinary team to ensure safe, compliant treatment and timely documentation.

Responsibilities include administering therapies, educating patients and families, and updating care plans as patient status evolves. Travel to patient homes is required, with supervision as needed.
