Apply your inpatient clinical expertise to review hospital documentation for accuracy, completeness, and consistency with the underlying patient encounter. This remote contract role is designed for experienced hospitalists who can identify clinically meaningful issues, resolve ambiguity, and explain nuanced clinical judgments clearly in writing.
Key Responsibilities - Review inpatient clinical documentation, including history and physical notes, progress notes, and discharge summaries, for clinical accuracy, completeness, appropriateness, and fidelity to the patient encounter.
- Adjudicate ambiguous, conflicting, or inconsistent clinical information and provide clear, well-reasoned conclusions.
- Identify clinically significant errors, omissions, and inconsistencies, and categorize findings by severity.
- Apply structured review rubrics and quality criteria consistently across cases.
- Provide concise written rationale supporting each adjudication decision.
- Contribute to the refinement of review guidelines and quality standards.
Qualifications - MD or DO degree.
- Board certification in Internal Medicine or Family Medicine.
- 5 or more years of experience as an attending hospitalist, or demonstrated leadership of clinical documentation audit programs.
- Strong expertise in inpatient medicine, clinical documentation, clinical adjudication, error detection, and documentation review.
- Deep familiarity with inpatient documentation standards and hospital quality assurance processes.
- Experience with clinical documentation improvement, or CDI, programs is preferred.
- Familiarity with how clinical documentation affects coding, billing, and compliance.
- Ability to work independently in a remote environment.
