Location: New York, NY 10016
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Weekly Blended Pay: $2,200–$2,415
Job Description
We are seeking an experienced Licensed Master Social Worker (LMSW) to join a hospital-based social work team. The ideal candidate will have strong experience in inpatient/medical-surgical settings, discharge planning, and care coordination.
Responsibilities
Participate in interdisciplinary rounds and collaborate with physicians, nurses, and healthcare teams.
Complete psychosocial assessments, care plans, patient education, and EMR documentation.
Provide discharge planning and support to patients and families.
Coordinate referrals for home care, skilled nursing facilities, rehabilitation, and community services.
Connect patients with appropriate post-acute and community resources.
Maintain accurate and timely documentation.
Utilize systems such as EPIC, Allscripts, Careport, and/or Parachute.
Requirements
Active NYSED LMSW license with Primary Source Verification.
Minimum 1 year of LMSW experience in a hospital, inpatient, acute care, or medical/surgical setting.
Strong discharge planning and care coordination experience.
Experience coordinating referrals to home care, SNFs, rehabilitation, and community resources.
Strong communication, organization, documentation, and interpersonal skills.
Ability to work effectively within an interdisciplinary healthcare team.
EPIC experience.
Inpatient medicine or surgery experience.
Experience with Allscripts, Careport, or Parachute.
Knowledge of community resources and post-acute care services.
