The Concierge Care Manager, East owns day-to-day coordination for active Concierge Care Collective clients and families across the East. You will be the person who knows where every piece of the plan stands: which appointments are set, which referrals are open, what the coach saw this week, and what the family needs to hear. You will work closely with our founders, our coaching team, and local providers, and you will be a steady, trusted presence for the families we serve in the region.
This is not a desk role and it is not a crisis hotline. It is a coordination role for someone who is organized, clinically literate, calm under pressure, and good with people.
Responsibilities
- Manage an active caseload of individuals in the East across substance use, psychiatric, and medical care needs
- Build and maintain each individual's care plan, care team roster, appointment schedule, and referral tracker
- Coordinate with providers across the region: therapists, psychiatrists, primary care, treatment programs, sober living, and specialists
- Build and maintain a network of trusted local providers and resources
- Review daily coach reports and translate observations into next steps, escalations, and plan adjustments
- Log regular case manager reports covering work performed, client status, provider and family contact, escalation level, and next steps
- Prepare weekly case updates for families and care teams
- Identify watch items early and escalate safety or clinical concerns to leadership promptly
- Support step-down planning as individuals transition from higher levels of care back to home and community
- Communicate with family members, trustees, and advisors with discretion, precision, and warmth
- Maintain accurate, timely documentation in Concierge Care Collective's systems
Qualifications
- Bachelor's or Master's degree in social work, counseling, psychology, nursing, or a related field
- Licensure preferred: LMSW, LCSW, LPC, LMHC, LCAS, RN, or equivalent, valid in the state(s) you serve
- 3+ years of case management or care coordination experience in behavioral health, addiction treatment, psychiatric care, or a related setting
- Working knowledge of levels of care, from inpatient and residential through IOP, outpatient, and recovery support
- Familiarity with the treatment and provider landscape in the East
- Strong organizational skills and the ability to manage many moving parts without dropping any
- Clear, professional written communication; comfort writing for families and for professional stakeholders
- Sound judgment on when to act, when to escalate, and when to simply listen
- Discretion. Our clients are often high-profile or private, and confidentiality is non-negotiable
- Comfort with technology and willingness to learn our systems (Monday.com, HubSpot, and related tools)
What Sets a Candidate Apart
- Experience in concierge, private-pay, or high-net-worth care settings
- Established relationships with treatment programs, psychiatrists, or recovery providers in the East
- Experience supporting families through interventions, discharges, or transitions of care
Engagement and Expectations
- This is an independent contractor (1099) role. Most work happens during business hours, with some evening and weekend availability expected for escalations.
- Regular local travel to meet individuals, families, and providers across the region.
